I now have screenshots from inspection of my records from therapy with Shane T. Raymond at Brandon Mental Health. The ENR documents there never was a treatment plan and he created three treatment plans after he quit and backdated them 2 years, 1 year and 6 months. Several progress notes were also altered after he quit two years later. I requested the original time stamped documents before they were altered but he refuses to provide a copy. BMH is a facility that lures people to come and receive services but at the end of the day all they care if for themselves. My boyfriend went there seeking for help in different issues that are affecting his present and maybe his future , his counselor interviewed him once then on the second appoinment she expressed her experience was not long enough to treat him, he called the administrator who did not help either and during the conversation she mentioned he could go to the “ Crisis Center” my boyfriend was not suicidal nei... read more

Provide a notarized certified copy of 178 pages of records that have been designated as the complete record set maintained by Shane Raymond and Brandon Mental Health, LLC, to me. Has been done, with the exception of the providing the notarization. 2. More correspondence I received wanting me to sign a release of all liability. I am in no way asking you to reward behavior. Far from it. I'm only asking so that all can walk away from THIS situation and so that you can feel protected. I can't change behavior for anyone for anything, but the hope is that experience guide actions in the future. That applies to everyone for all things. Shane had the opportunity for over 2.5 years to privately correct the records he created (fabricated), backdated and altered and provided to numerous third parties after he quit. But he, his attorney or both chose to drag it out and cause severe distress. What he did has to be the most selfish behavior I have ever seen and not the behavior I would expect from a social worker who is supposed to help people and not cause them harm. He now voided the records but because he fabricated and altered the records they no longer match... read more

After 3.5 years, Shane Raymond was required to appear before the Board of Social Work. He will be on probation for 1 year with record review by a supervisor, pay a fine, costs, and take continuing education courses. He has now voided 64 pages of my records including treatment plans that he created after he terminated and backdated them. There never was a treatment plan the entire two years of therapy. He has yet to correct all records. Not the qualities I would expect from a LCSW or the CEO. Be careful and ask to inspect you... read more

I sought help for PTSD from SA trauma. I came to therapy already diagnosed with PTSD. Instead Shane ignored and diagnosed generic depression. He claims there will be an accurate diagnosis but somehow he ignored for two years why I was seeking help. He claims there will be an individualized treatment plan. Not only was a plan not individualized there never was a treatment plan the entire two years I was a client. After he quit by email while I was in crisis he then fabricated three treatment plans.
