My husband is deceased and my daughter and I are fortunate to have remained on his insurance, NYSHIP. Beacon Health was its mental health division (which we needed in our grief.) However since Carelon acquired, I received denials. First, because the policy is in his name and proof of payment by him must be shown - even tho they “have a note he is deceased.” Carelon sent a form to make me Alternate Payee.
