Good to be back under your care! Due to an insurance coverage miss communication I am not sure our primary & secondary insurance is recorded properly In your database. I am requesting that the pre pay I did for my services scheduled in May be credited back to my Visa, then recalculated for the actual charges with correct insurance coverage be paid upon receipt of services.
