I saw therapist Jason Willus at Whitestone Counseling. When I requested my records, Leah Leonard (LMFT #42993), who handles documentation, sent me a poorly formatted Word doc from a Yahoo email. It had no official letterhead, no provider signature, no patient name or ID, and no service dates — making it both medically and legally invalid.
