Yesterday I visited my primary physican On 2/11/13 i saw my primary care physican for clearence to have surgery 2/22/13, I needed ekg, renewal for hctz , mobic, signature for surgery , I had lab work done at PRMH, and i requested a permament disable card, I would like for any body to check my records and see how many times i have been in and out of that office for my care.
