Update- finally they sent the form but of course waited until they closed to send it so now I have to wait until next week to get my records. Absolutely unacceptable the way I was spoken to and the lack of urgency especially when I told them my hardware is loose and I need the records asap. The amount of pain I’m in and the way they are treating is absolutely unheard of when all they had to do was take a few mins to email me records. Highly don’t recommend this clinic.
