My son needs his to get a physical exam in order to play sports this summer. His new Dr. needs his records in order to take the physical but i have no way of getting them. I've called, went to the offices and no luck. i see that some people were able to obtain them. can someone lead me in the right direction. thanks
Hello...I saw someone posted 2 weeks ago they were able to get their child’s records from Dr. Tolentino. Her phone number isn’t working. How did you get in contact as i need my child’s records. Thank you for any help in this. I’m sad to hear she retired.
Edit: Was able to contact the doctor and got all the records. Thank you doctor. My kids vaccination records are with the doctor,is there anyway I can get it.i need it for his school admissions.i am trying to reach her on the phone mentioned in the website but it is not connecting.Any advice would be helpful.thanks a lot.
I love this doctor! Dr Tolentino has always been there when I needed her for my kids. I’m sadden to know she has retired. She was very nice and professional with explaining things. My family and I love you and want to say Thank you for those Great 14 years. God bless! Love Jeremiah Ramos mom ❤️
