I received a call to schedule my appt and was told I’d get some HIPAA forms and regular patient intake forms in my email. Confirmed the email address with whomever called but NEVER got any emails from them AT ALL! I had a 9am appt on 1-3-23. Went to the address given by the referring Dr as well Google and a text sent by the Therapist. The location was listed as an acupuncture and wellness center.
