My saving grace!!!
I dealt with Nasra who now refuses to give me information to contact the owner of the office. She was rude as I told her I got the form and confirmed I was filling it out told me since I wouldnt fill it out my appointment would be rescheduled. Literally while I filled out the line for preferred language. I explained I would have filled it out before had I gotten this email before 10 minutes ago.
