I needed to change my primary as I moved to Lake Worth Beach .. I set an appointment for 3:15pm and gave the office over the the phone all my health insurance and other info.. I walk in and check in and I tell the woman that I am a new patient and ask if I need to fill out any paperwork which both offices make you do a number of pages.
