I seen a huge roach crawling on the floor. and the signs out front on the building says they do x-rays and I walked in and the woman told me they don’t do x-rays which doesn’t make sense. I came in 2 days in a row for an appointment after a work incident and they said they needed an authorization form when my job sends the forms over for every work incident and they’ve never had this problem.
