Dr.DylanHagerty at Smile Design Dentistry, West Chase location provided horrible service. On 12/18/ 017 Dr Hagerty performed root canal treatment, followed by porcelain crown installed on 1/5/018. The porcelain crown was installed without any pins and failed (fell off) within first 3 month. Office refuses to take responsibility for providing sloppy. failed job.
