Falsified report – Went to this Quack for a determination on condition, which is documented previously, and he lied on report with regard to my condition. He as well put down on my report, I smoked cigarettes, (I don't), he put down I drank, (my sobriety is not a thing to play with), and that I drank three cups of coffee a day.
