About to get a warrant issued because of no response. I have requested a note stating that I cannot physically travel and sit in for jury duty. I was told I was going to get it after my checkup, I did not. I called in, was told I was going to get a call back, I did not. I sent in an email, I got the visit summary, no note. I tried following up via email multiple times, no response. I tried calling back again and again, and every time, I am told I will get a call back, with no call back.
