Challenges with referral process. Called 3 times. Facilitated referral through PCP & the Moline office. Called 3 times to see if they had my referral information, they did not. Called morning of, still no referral info. Called PCP, refaxed it. Arrived 30 min early as directed to fill out paperwork. To then tell me I didn't need to complete the paperwork they'd just complete it in the room. The initial assistant only wanted to focus on my family history.
