Did this provider have reasonable prices? Absolutely! Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Did they follow up with you after your appointment? Yes, they made sure I had everything I needed! Did this provider promise services he/she couldn't provide? Not at all! They exceeded my expectations Are you confident that this provider will continue working with you until a solution is reached? Absolutely! I'm confident they will work hard to find the best solution for me

Did the staff make you feel uncomfortable when you called with questions or concerns? Not at all! They were happy to address every concern I had Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of Does this provider give unbiased advice? Yes Is this business in a safe neighborhood? Yes, it was in a safe enough neighborhood

No, they were able to provide everything they said they would Did this provider ever take personal phone calls while in the appointment room with you? No, they've never taken personal calls with me Did this provider seem up-to-date with the current advancements in their field? Yes Were the staff members able to answer your questions? Yes, they were able to answer my questions

Did this provider's staff seem to respect him/her? Definitely! They staff spoke very highly of him/her Did this provider listen to your input and concerns? Absolutely! This provider always listens carefully to what I have to say! Were you able to relax during your appointment? Yes, I was completely at ease

Yes, I was completely at ease Was the waiting area spacious? No, but I can always find a seat Did this provider have reasonable prices? Is this provider willing to pursue advice from other providers when necessary? Absolutely! They aren't arrogant at all and are more than willing to pursue advice when necessary Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away Has this provider ever cancelled your appointment on you last minute? They try not to, but sometimes it happens. Was this provider's office clean? Yes everything was neat and sterile
