Did this provider ever take personal phone calls while in the appointment room with you? He/she would never do that! Their full attention is always on me. Does this provider promptly return your phone calls? Definitely! They always call me back very quickly! Do this dentist have a good professional reputation among other dentists? Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Was the chair you sat in during your dental procedure comfortable? Yes, it was comfortable Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute

Did you leave the office feeling satisfied with your visit? Completely! Did this provider seem irritated to be working with you? Not at all, they were glad to help me with anything I needed Did you spend a lot of time in the waiting room at this provider's office? Absolutely not! I was seen right away! Does this provider maintain a professional appearance? Absolutely! You can tell that he/she really makes an effort to look professional

Does this provider maintain a professional appearance? Yes, he/she looked very professional Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Does this provider welcome questions? Absolutely! They are happy to answer all of my questions Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Did this provider seem irritated to be working with you? Did this dentist allow you to regularly rinse your mouth during your procedure? Yes, I was able to rinse Did you have to wait long after you arrived for your appointment? No, I didn't have to wait very long Do you feel that you could have received better service somewhere else? No, I don't think so Were the restrooms clean at this provider's office? Yes, I could tell they were cleaned regularly Were the staff members able to answer your questions? Yes, they were able to answer my questions

Were you provided with payment plan and fee information before your dental treatment was scheduled? Absolutely! They explained everything in detail and made sure I understood before my procedure
