My mother has been there two times for rehab and got great care. Took her back last week and things have certainly have changed, liked to have never got her out of there, we were deceived and not told about the 14 day quarantine. 91 year old did not understand what they were doing to her she cried her eyes out, fourteen days of quarantine and not getting rehab in rehab room was not an option. No one would return our calls to get her moved out of there very poor mangement.

RecommendedMost of all staff is friendly and attentive to your needs. Social Worker is on top of your needs and handles all your paper work. Nurses stay abreast of your meds and always pleasant. AIDS are extremely friendly and always there to help. Physical therapists are atop their duties yet try to make it a pleasant experience.

dietitians very good food was good one time when we had baked fish it was overcooked 0 physical therapy was real good took their time nurses were very nice

there were 3 nurses that I had the pleasure of knowing there. however the cleanliness, and the condition of equipment was abhorrent! the food was never right. neither in description, or preparedness, always out of something,. ability to get "snacks" was iffy. at least 2 medication errors. the lab used is either inefficient or needs to calibrate machinery,. I wish the best of luck to anyone who has to be there long term. it is not really fit for rehab.. Dont get me started. was terribly disappointed. Chose because only place ... read more
Providence Park in Tyler, TX is absolutely horrible! I am on my up there today to talk to someone over the facility after trying to reach someone for several minutes to set up a window visit for my dad. A rude lady finally answered the phone by saying “hello!” While trying to explain what I wanted she asked “is you dad even here?” 😡 At this point my blood is boiling because clearly my dad is a patient there or I wouldn’t have called. Employees are always there for you for whatever need you may have, kind, professional, willing, spreading cheer. Facility clean and easily assessable. Aides are always there for you whatever the need. Love them and nurses for their service.
