the Patients Like Me is so cool in Helping People that why I Like it and Thank you for having it to.
It asks for an awful lot of detailed information with dates, which I'm not totally sure of and I got disrupted before I completed. This would be a good way I hope, to match up my situation with treatments , support people and others in my situation I would hope.
