It makes absolutely no sense to hand the patient a pen to fill out paperwork. Suggest patient use their own pen if they have one. Or Offer a pen from a disposable paper cup - the patient can then take the cup to their room. The assistant can then pour the hydrogen peroxide mix into the same cup.
