If I could I would rate Omni -2. The dentist I wanted was in my insurance list of providers. So I made an appointment with her in one of her Omni locations. I went to my appointment. The questionnaire probed me with some very personally invasive questions about my sexuality and what I did for a living that I found totally unnecessary.
