Was the Dental Care provider able to take and develop x-rays in their office? Yes Will this Dental Care provider try to get you an appointment ASAP if you have an emergency? Absolutely Did this provider take time with you at your appointment? Yes Did the toothpaste used during your cleaning taste OK? Absolutely Are you confident that this provider will continue working with you until a solution is reached? Absolutely

Yes, they went above and beyond Were you glad that you had the appointment with your provider? Yes Did the staff call you to remind you about your upcoming appointment? Yes, they called me a few days before my appointment Was the toothpaste used during your cleaning enjoyable? Yes

Did you receive sample toothbrushes or toothpaste to take home? Yes, I received sample toothbrushes and toothpaste Did this provider give the impression that they cared about your concerns? Yes Did this dentist provide helpful oral health techniques you could practice at home? Yes Was this dentist's staff friendly? Yes Were the waiting room chairs comfortable at this dentist's office? Extremely comfortable; I could have fallen asleep

Were the staff members able to answer your questions? Yes, they were able to answer my questions Was this provider's office clean? Yes! I was amazed at how clean it was! Was this provider argumentative or easily angered? No, they were very calm and understanding Was the water they used during your teeth cleaning a comfortable temperature? Absolutely! It was the perfect temperature Does this provider give unbiased advice? Yes

Did the staff call you to remind you about your upcoming appointment? Yes, they called me a few days before my appointment Was the chair you sat in during your dental procedure comfortable? Yes, it was comfortable Did this dentist explain your dental procedure to you? Yes. They made sure I understood every single detail Did this provider seem well-trained and experienced? Yes, they seemed very qualified Did this provider ever postpone your appointment? Not at all! They always try to get me in as soon as possible
