Do you feel that you could have received better service somewhere else? Definitely not; I received the best service ever! Did you experience unnecessary pain during your visit? Not one bit! I felt much better when I left! Was this provider's office too loud? Not at all, it was very peaceful and quiet Does this provider use current and safe equipment? Fairly current Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute

Does this provider promptly return your phone calls? Yes Was it easy to make an appointment with this provider? I waited longer than I wanted to, but it wasn't terrible Do you feel that you could have received better service somewhere else? Yes! I know that I would have gotten better service anywhere else! Does this provider welcome questions? Not really. I felt like an annoyance when I asked a question Is this provider's business conveniently located? Yes, it is very conveniently located
