Was using dr.DeClark but am choosing another primary care physician. Getting a prescription Refilled is like pulling tooth and nail! You will wait for days and days and can never reach anyone at the office at the rogers location. The phone just rings and rings and rings. My overall experience isn’t horrible but I wouldn’t say it was a good enough experience to want me to come back.
My name is Shane Henderson. I have been a patient of MR Declerk for over 2 years. He goes above and beyond for myself and wife. I would wait 3 hours to see him over any other doctor in NWA. I can’t thank him enough for what he has and continues to do for myself and family.

I had the worst experience from Northwest at this clinic. There is only one doctor and he was 55 minutes late for an appointment. They ask you to be there early to have time to fill out all their paperwork and then you wait and wait. To add insult to injury, he skipped my appointment and went to the person who's appointment was after mine. AND the office manager had no sympathy my situation. She said she processed my refund but wouldn't print out anything that showed it had been even requested. The judgement in her eyes that... read more
