I had always heard that this was a great place for rehab. IT IS NOT!! I had knee replacement and stayed there 6/14/25 through 6/18/25. It was a horrible experience....pain medication was 1-2 hours of waiting after asked for. At admission the nurse was told that orders had been given that I have a heart monitor, and that the Dr.'s office would deliver it. NEVER GOT IT. I reminded them twice. I actually had to ask for water daily. Sopping wet towels were left on bathroom tiles all day...I finally kicked them out of the way so ... read more

Our experience was that the medical team (2 NPs and 1 doctor) is slow to act on diagnosing issues. It took 5 days after my mother started suddenly having delirium to do the first tests to see if there was a medical reason. Certain staff members did not follow orders for care. We could not get any information from the NPs during this health crisis. Instead, we had to chase after nurses to find anything out. If you want your loved one to continue to live, DO NOT put them in this facility. Even the Rehabilitation Section is dangerous. There is literally no care at all. ZERO. No one is going to assist them with baths, eating, dressing, changing under garments/adult diapers, etc. Your loved one will develop bed sores...terrible bed sores at least grade 1. They will wait hours for meds, hours for help with anything. They will sit in a wheelchair all day and not be allowed to get back to their bed until 6 hours have passed. Yes, the... read more

I have had a great experience with NHCof Sparta tn. Michelle pack, Jennifer Selby and all the nursing staff and transportation team as well as brook and her therapy team had treated my dad so well and Ben over backwards to care for his every need. They are great!

My father has been in this facility for 4 months. He brought it to the attention of leadership that a bed bug infestation broke out in his room. They failed to act after 2 weeks after telling us it wasn't bed bugs. Blood spots were all over his pillows, headboard, and mattress. We had a conference call with the director, and we could tell she didn't want this going beyond her. The first night, they put him on a blow-up bed in a spare room. He fell out of bed. ALL the staff are Awesome - helpful, compassionate, attentive, gentle, collaborative and knowledgeable. The “food” would KILL anyone’s health and mind!!

This place is horrible! I have a friend who is currently there. Has been there several years. It was a nice place several years ago, but has since gone down drastically. The majority of the CNAs really don't care about the residents. Too busy at the nurses station on their phones. My friend is currently listed as hospice care, wanted a banana to eat and was told to wait until dinner since she just had half a banana couple hours earlier for lunch.
