Was the Dental Care provider able to take and develop x-rays in their office? Yes Were you satisfied with your appointment with your provider? Yes Do you think this provider would be willing to pursue advice from other professionals if they run into a snag in your treatment? Probably Was the noise level reasonable in this provider's office? Reasonable Did this provider seem well-trained and experienced? Yes

Mostly yes Is this provider willing to pursue advice from other providers when necessary? Probably not Was the water they used during your teeth cleaning a comfortable temperature? Not really Would you refer this provider to a family or friend? No

Does this provider offer any coupons, discounts or promotions? Not that I have seen Did this provider pressure you to purchase any unnecessary products during your visit? Absolutely not! I was never uncomfortable How would you compare this provider to others in his/her field that you have visited? Among the best I have seen Was the dental equipment clean? Absolutely! It looked brand new Did a staff member acknowledge or greet you when you entered? Yes, I was greeted when I walked in

No, if they had music on it was very soothing Was it clear that this provider takes pride in the appearance of his/her office? Was this provider's office too loud? Way too loud! I could barely hear what anyone was saying over the noise Has this provider ever cancelled your appointment on you last minute? They cancel on me all the time. Does this dentist use current and safe equipment? Not at all; the equipment is dangerous! Did this provider have an entertaining selection of waiting room magazines? Did you have to wait long after you arrived for your appointment? No, they were ready for me right after I arrived. Did this provider answer all of your questions? Every question I had was answered thoroughly Does this provider offer any coupons, discounts or promotions? Not that I have seen Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Did this provider ever postpone your appointment? Not at all! They always try to get me in as soon as possible
