First off - "Please call our office at (601) 645-1523 to schedule an appointment." The phone number is not in service!! Huh?! Secondly - I finally found a working number and made an appointment. In turn, they emailed me a whole lot of forms to fill out which I am to bring with me at my appointment. I show up with all the forms filled out and handed them to the lady at the window.
