Were you able to watch TV during your dental procedure? No, but I was able to read a magazine Was this provider's office too cold? Not at all, it was the perfect temperature Did this provider ever take personal phone calls while in the appointment room with you? He/she would never do that! Their full attention is always on me. Did you notice a foul odor when you arrived at this provider's office? No, I didn't notice an odor

No, they never postpone my appointment Did this provider use excessive foul language? Not at all! They are always respectful and courteous! Did this provider promise services he/she couldn't provide? Not at all! They exceeded my expectations Did this dentist give you your invoice promptly? Absolutely! They gave me my invoice before I had even asked for one

Does this provider treat their staff well? Very! I've never seen an employer treat their staff better Did this provider listen to your input and concerns? Yes Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Did this dentist give you your invoice promptly? Absolutely! They gave me my invoice before I had even asked for one Was this provider's office too cold? No, it was fine

Are you confident that this provider will continue working with you until a solution is reached? Yes, I know I can count on them to find a solution Was this provider's office easy to locate? Fairly Easy Did this provider wear an obnoxious cologne? Not at all, their cologne smelled great Did you experience any unnecessary pain while your teeth were cleaned? Not at all, they always make an extra effort to make sure I'm comfortable Was the waiting area spacious? No, but I can always find a seat

Does this provider leave you on hold for a long time when you call? Never! I don't think I've ever been put on hold here for a long time Did this provider show attention to detail? Yes, I've never had to worry about them missing any important information Did a staff member acknowledge or greet you when you entered? Immediately! They gave me a friendly greeting as soon as I walked in Did your mouth feel clean after your appointment? Do you feel that you could have received better service somewhere else? Definitely not; I received the best service ever! Did you feel like this provider's office was understaffed? Not at all, they had plenty of staff members to help me whenever I needed assistance Was this provider's office easy to locate? Absolutely! It couldn't have been easier! Does this provider always put the customer first? Absolutely! They have some of the best customer service I have ever seen
