They are not up front when it comes to insurance. They will not call to verify your insurance and will make false claims that its 'expired' making you pay cash for initial visit. Suggest calling ahead before wasting time filling their online stuff out and driving over for an appt. Editing to answer you. I filled out the form for my boyfriend a year ago.
