I was referred to this office by the hospital as a primary care option after an ER visit. When I called to establish care, I was told there is a required $2,500 annual membership fee in order to become a patient. While I understand some practices operate under a concierge or membership-based model, I wish this had been made clear upfront before being listed as a primary care referral.
