Did this provider answer all of your questions? No, I left confused and frustrated Does this provider offer any coupons, discounts or promotions? Not that I have seen Was this provider's office easy to locate? Fairly Easy Were the staff members able to answer your questions? Yes, they were able to answer my questions Did you have to wait long after you arrived for your appointment? Yes, I had to wait quite a while

Yes, they don't mind answering my questions Would you refer this provider to a family or friend? Yes Did this provider have reasonable prices? Yes Was this provider's office too cold? No, it was fine

Yes, they were able to do almost nothing they originally said they could Is this provider willing to pursue advice from other providers when necessary? No, and they didn't want me to seek anyone else's opinion Did this provider seem well-trained and experienced? Yes, they seemed very qualified

Is this provider easy to reach in an emergency? Absolutely not. I was forced to call the emergency room. Are you going to visit this provider again? Only if I couldn't find a better substitute

Did the staff make you feel uncomfortable when you called with questions or concerns? Absolutely! They made me feel like my questions and concerns were stupid Was this provider's staff friendly? Yes, they went above and beyond Was it clear that this provider takes pride in the appearance of his/her office? Yes, the office was well organized and taken care of

I Already Have Was the waiting area spacious? No, but I can always find a seat
