All my previous experiences here were great. However, today my experience wasn't so grand. I came for a cleaning, normal routine stuff. They asked me to complete a full intake form. They said it was required by insurance. Maybe that's my crappy insurance. However, nothing on the form had changed from the first time I filled it out. Why do you expect me to sit there for 30 minutes filling out a form when nothing is changed. Why can't you just give me a form that lets me say if anything has changed.
