I have been trying since Dr Hassan told me he was closing his office to get my medical records with no success. He is such a great physician so disappointed to learn he is with the VA so I can no longer see him; however he told me I could make an appointment with his father in law however I never was able to find out the father in laws name or practice phone number.
Tricare says we can go here... Called and says, "PERMANENTLY CLOSED" thanks for world class service TRICARE
