Did you leave the office feeling satisfied with your visit? Completely! Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away Were you provided with payment plan and fee information before your dental treatment was scheduled? Absolutely! They explained everything in detail and made sure I understood before my procedure

Did this provider ever postpone your appointment? Yes, it's usually postponed at least a week Did you feel safe in this provider's care? Absolutely Not! How would you compare this provider to others in his/her field that you have visited? The worst I have seen Was this provider's office too cold? Yes, I was freezing

Was this provider's office too cold? Not at all, it was the perfect temperature Did this provider have reasonable prices? Yes Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable. Were the staff members able to answer your questions? Yes, they were able to answer my questions

Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Does this provider always take that extra step to make you feel special? Absolutely, they always make me feel like I'm their only patient. Did the staff make you feel uncomfortable when you called with questions or concerns? Not at all! They were happy to address every concern I had

You can tell that he/she really makes an effort to look professional Does this provider leave you on hold for a long time when you call? No, they are really great about answering the phone right away Did this provider promise services he/she couldn't provide? Not at all! They exceeded my expectations

Does this provider offer any coupons, discounts or promotions? Not that I have seen Were you provided with payment plan and fee information before your dental treatment was scheduled? Not really, they gave me a little information beforehand but not very much Was the water they used during your teeth cleaning a comfortable temperature? Yes, it was fine

Somewhat, they eventually called me back Did you receive sample toothbrushes or toothpaste to take home? Yes, I received sample toothbrushes and toothpaste Was the water they used during your teeth cleaning a comfortable temperature? Yes, it was fine Did this provider leave you unattended for an extended period of time? I was left alone for a while, but it wasn't too long

No, it was fine Does this provider give unbiased advice? Yes Did you experience any unnecessary pain while your teeth were cleaned? No Did your mouth feel clean after your appointment? Definitely! It felt more clean than it's ever felt!
