Terrible business practices. Anybody can call an ambulance on you for any reason - even if you don't need, want or use them - and they will bill you, not the caller. DO NOT sign anything - they will tell you it's an AMA release form (since you don't want them and didn't call them, why would you go with them) but what they don't tell you you're signing is a promise to pay them if your insurance doesn't.
