To whom it may concern, Mrs. F***** was seen for an initial exam on January 8th of 2020. We completed a thorough exam which included x rays and a perio chart. The perio charting indicated that she has moderate to severe periodontitis with gum pockets as deep as 8mm in the upper left quadrant amd 7mm pockets in the lower right quadrant. The best way to treat this disease is with scaling and root planing. At the time of her initial she did not have dental coverage so we provided a discount plan for her which reduced the price from $1820 to $1095, because again she did not have dental coverage. We offer a $59 special that includes the exam, x rays, and a basic cleaning. She signed a Notice of Perio Consent to decline the scaling and root planing, and we did the basic cleaning which was explained to the patient that this would not do any good for her condition. She then returned to our care in September and was reminded of the treatment the doctor recommends and she was not happy. To Mrs. F*****: I do apologize for any inconvenience that may have been caused when you returned to our care, I would love to have you come back for a basic cleaning.
