Did you leave the office feeling satisfied with your visit? Yes Was this doctor friendly? Yes Did this doctor answer all of your questions? Every question I had was answered thoroughly Was this doctor's staff friendly? Yes, they went above and beyond Does the provider's staff try to be helpful? Yes

Does this provider treat every client equally? Definitely, they make everyone feel equally respected and cared for Does this provider adhere to the customer is always right policy? Absolutely, the customer is always right here Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Would you refer this provider to a family or friend? Yes, I'm going tell everyone about them. I absolutely love this place! Was this provider friendly? Very! The most welcoming office I've visited!

I was very satisfied with the care of the Doctors, and individuals that took my blood. I never noticed any smells. The wait was very long and there is usually not enough chairs to seat everyone.

Is this provider's business conveniently located? Yes, it is very conveniently located Did this provider ever take personal phone calls while in the appointment room with you? He/she would never do that! Their full attention is always on me. Did this provider wear an obnoxious cologne? Not at all, their cologne smelled great Were you able to relax during your appointment? Yes, I was completely at ease Was this provider's office clean? Yes everything was neat and sterile

Is this business in a safe neighborhood? Yes, it was in a safe enough neighborhood Did this provider ever dismiss your concerns as unimportant? Never, they go out of their way to address every concern of mine immediately Were the staff members able to answer your questions? Yes, they were able to answer my questions Is this provider's business conveniently located? Absolutely! It is right off the freeway and close to everthing Did this provider seem well-trained and experienced? Yes, they seemed very qualified

Has this provider ever cancelled your appointment on you last minute? No, they didn't cancel on me last minute Is this provider's business conveniently located? Yes, it is very conveniently located Did this provider thoroughly explain the risks and benefits of your treatment? Yes, I felt informed when I left Was this provider late to your appointments? Sometimes, it was a little frustrating Was this provider friendly? Very! The most welcoming office I've visited!
