On September 3, a staffer was taking my mom to the restroom in her Unfortunately, my mom foot came off the foot holder as the staffer continue pushing her and she fell out the wheelchair face first. My mom was examined and has a bruised nose. One day has gone by, and i have not received one telephone call from the center to check on my mom. Unacceptable. This incident could have been major for a 84 year old lady.
Don't always do hygiene checks during the day.
Smells of urine upon entering the facility
I really wish they'd check their public info. The phones ring off the hook and say it's disconnected. :( Unfortunate b/c I'd really like to speak to someone there. Some inquiries can't be answered via a website. :(
