Was this provider's staff friendly? Yes Did the staff make you feel uncomfortable when you called with questions or concerns? Absolutely! They made me feel like my questions and concerns were stupid

Does this provider maintain a professional appearance? Yes, he/she looked very professional Was this provider late to your appointments? No, they were on time Does this provider ever overbook appointments here? Sometimes, they should allow for more time in between appointments Does this provider treat every client equally? Yes, they don't show favoritism Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would

Yes, they went above and beyond Did you leave the office feeling satisfied with your visit? Completely! Was this provider's office easy to locate? Absolutely! It couldn't have been easier!
