Purpose of visit is to establish care with my new PCP Just moved here after living 60 years in Pennsylvania. I’ve enjoyed establishing a relationship with Dr. Lavender and I look forward to her care in the future.
This was my first visit with Dr. Lavender. She addressed my health care concerns and provided me with some information on how to best manage my health. I felt at ease with her.
