Dr Middlebrooks has been my primary physician for several years. He has always been very quick to respond to my questions or medical needs. He is personable, professional, dependable and very much appreciated.
I am a new patient. I just had my 2nd visit with Dr. Middlebrooks after having to find a new Dr to replace my previous one who I had for 20 years. I am SO pleased to find Dr. Middlebrooks. I felt heard and understood. He answered my questions informatively and explained different options for treatment. I felt as if I learned a few things. He was patient, considerate, friendly, and encouraging.
Dr. Middlebrooks has been my doctor for several years. He is genuine cares for your well being. Does not just stop by to do a few things and go. He takes the time to explain what’s going on with my medication and exams. I highly recommend him as a physical.

Dr. Jack Middlebrooks spent ample time with me to answer all my questions. Last year there were a few very significant findings at my annual exam. I wanted to be sure this was a thorough exam. He went through my blood test data in great detail. I felt very confident that the physical exam was thorough.
He has been my husband’s primary since a child and we base our insurance on him because he is the best. He takes his time and truly cares about his patients. I recently have gone to see him and he does his best to accommodate and listen too all your problems. I trust him and would recommend to family and friends.
