Years ago, I went into the Fort Myers location on a special marketing offer. After that day I tried to confirm that the doctor they were trying to set me up with was actually in-network. They kept telling me she was and they kept calling me to schedule an appointment. However, my (major) insurance company kept telling me she (the doctor) was not registered with them and she needed to fill out a form and submit it to them.
