I contacted Innovative because they were independent and covered under my insurance provider. Innovative contacted my insurance company to verify my annual free physical would be free. Innovative sent my blood work TO AN OUT OF NETWORK LAB and my cost (after discounts of a couple hundred dollars) is 1300 USD, THAT I DONT HAVE. I contacted my health insurance provider (Humana).
