I hate to say this but this was the worst post Acute hospital I have ever survived. Staff had no interest in patient care. No uniforms. I now go to Sage Creek. That is new very nice. Great nurses. Good food. Private rooms. Horizon is older. Appears to be unclean and crowded. I was jammed in on a gurney at the foot of the other patient in what was previously a room for one. Meds were haphazard and intermittent. Never saw a doctor.

This place is sketchy to say the least. You are lucky if somebody answers the phone when you call. They made it impossible to plan for long term care with my father, as they did not keep me updated on his status or the conditions of his insurance coverage. He suffered from dementia and kept climbing out of bed and falling down. They did nothing to rectify this. Just waiting to see how big of a bill they send me for neglecting him!

Now I'm faced with either running up a huge bill or having him thrown out on the street. I will change my review if they rectify this situation.

The food has no nutrients. food is bad nurses were wonderful the managment do not seem to worry about the nutrition. staff was intelligent but not quik to respond. The beds are old and make noies that sound like its growling. the staff seems not to worry about quick response. we are not here for fun and need to be more closely looked upon and take care of
