There is no room in this survey for comments. I do believe the intake forms that you request for State compliance need a thorough redo. Multiple requests for the same info, name, address, phone, etc are made on the same sheet of paper. The privacy notice states something like " we don't need to abide by this, but if we do.... " But service and expertise are excellent throughout your office. Thanks! Todd Swanson.
