On the Order Form for the doctor to fax to your business there is no input box to annotate refills. There should be an extra column added on the form for the doctors to annotate how many refills. This has caused confusion in the past when submitting your required forms. I filled out the forms gave it to my doctor and he faxed it to your office. Without any orders, my Credit card is charged from CA. three times (and declined) and got marked as fraud. Once again, my card in use via the internet get hit with fraud.
