Nurse Stephanie inserted a catheter in the urethra, not the Bladder, and inflated the balloon. There was bleeding and no urine coming into the bag.She connected the catheter to the leg bag with no connector and stretched the catheter so the leg bag was on the lower leg. When the bleeding, lack of urine output and stretching of the catheter was brought to her attention she moved the leg bag to his knee. I asked how any urine would drain that way? Many mistakes were made. We had respite care on a few occasions, medication orders weren't correct. Lack of communication was consistent throughout the whole process. I had a phone triage person diagnose a breathing issue as agonal breathing (over the phone) telling me my husband would be dying that day, but that I should already know that. He wasn't, it was stridor breathing and later told me the triage person didn't realize he was conscious (never asked).
