Best dental visit I've ever had. I'm scared to death to go to the dentist and they totally put me at ease and they understand my fear.

Concerning question #4, the implication is if I wait less than 15 minutes, I did not have to wait past the appointment time. For the record, I DID have to wait, but it was probably less than 15 minutes. You might consider revising your survey to get more accurate information.
