I thought he was a excellent doctor untill he makes a mistake and writes the wrong form and then when i get him the right form he tells me he is not filling it out for me! And you need to wait months for an appointment and the oleny office looks like a dumpster! I am very disappointed in this man right now! He made the mistake of filling out wrong paper, his office lost the right papers and know that i got them again he noloner will fill them out for me
