I had an exceptional experience at Form MD. The front office was so nice and the staff was so welcoming. Diamond took time to show me the office and the treatments that they offer. Tami did a great job with my injections and was so friendly and professional. I look forward to my next visit.
Form MD is the place to go to if you’re thinking about diving into anything plastic surgery or aesthetics. I make the commute from LA to OC because I know I’m getting the best care possible. The staff is always extremely friendly! They welcome you with a level of professionalism that you want to experience with any physicians office you visit. I came in originally for a quick Botox treatment with Tammy and haven’t gone elsewhere since. 10/10 recommend!
Every visit I have to this office is pleasant and the staff is always extremely friendly and welcoming. Went for a quick appointment for a lip flip with Tammy and had an amazing experience. Can’t wait to see the results! Will definitely be coming back for more treatments :)
Tami is fantastic. First time I have not felt needles for Botox. She is honest and doesn’t oversell products that really won’t change your appearance.
Tami helped me with a ipl facial after I had lots of bruising from a blepharaplasty. Wow. The bruising started going away instantly that day and was back to my normal skin 2 days later. This office is so caring for there patients.
