Tami is amazing! I got a refresh with xeomin for an upcoming trip. Wonderful experience here and such a gorgeous office too!
This Review is not based On Tami, the nurse practitioner who injected 1 ml of Radiesse in each hand. This review is a heads up on their front office business practices. I was told it would to be $1400 Total. When I checked in the next day, I was told that $1550 was the best they could do. I had the best experience with consultation with Tami who was so patient! Tami did not try to sell anything she really explained every treatment and let me decide what is best for me. Tami even told me if something was good for me! I really appreciate that!
I’m new to the area and was introduced to Form by my boyfriend and am very happy. Tammy was great about recommendations on my injections and made me very comfortable. The office is super nice and everyone that works there is professional and make you feel very comfortable. I will definitely continue going for all my aesthetic needs.
